If you would like to request a transfer of your medical records from TMI Sports Medicine, the process is straightforward. Please follow the instructions below to ensure your records are securely transferred to Apex Orthopedics:
Online Request for Record Transfer
- Visit: https://requestmanager.healthmark-group.com/register
- Enter your email, and detailed instructions will be sent to you.
- After receiving the email, follow the link provided and click on "Submit a New Records or Forms Request."
- Answer the question "Are you the patient?" by selecting "Yes" and enter your personal information.
- Enter the following information for the facility that currently holds your records:
- Clinic or Hospital Name: TMI Sports Medicine
- Phone Number: 817-419-0303
- Address: 3533 Matlock Road, Arlington, TX 76015
- Under "Requester Information," please enter your personal details.
- For record disclosure, please provide the following:

- For record type, choose "Medical (all)."
- Review the privacy notices and sign by entering your name.
- Confirm and submit your request.
